The paramedian approach shown in the inset and in the posterior view requires an additional oblique plane to be considered, although the technique may be easier in patients who are unable to cooperate in minimizing their lumbar lordosis. The paramedian needle insertion is made 1 cm lateral and 1 cm caudad to the caudad edge of the more superior vertebral spinous process.
