WHEN TO DISCONTINUE THERAPY

The decision to stop resuscitation is a personal one. It is usually based on the physician's experience and the desires of the parents. In making the decision, the physician must consider the probability of neurologic damage and the likelihood of even semi-normality. If the chances of a productive, useful life are poor, consideration should be given to discontinuing all resuscitative efforts. There has been much discussion about whether to resuscitate micropremies.[108] [109] The outcome of neonates born before 26 weeks' gestation is abysmal,[110] [111] Consequently, there must be a frank discussion with the family before resuscitation of these neonates begins. If not, it may be necessary to begin resuscitation and then discontinue it after talking with the parents.
